Mayhem was a hologram and computer virus who served as an Intelligence officer in Starfleet in the late 24th and early 25th century. (Star Trek: Phoenix-X)

History

Mayhem joined the crew of the USS Phoenix-X in 2384. He was created by Section 31 to do the work of surveying and inspecting Section 31 influenced starships. When Mayhem inspected the Phoenix-X, he became infected by a portion of the Iconian virus, which was kept in a device taken from an Iconian probe years before. The result was a change in his personality and a darker attitude. He cut his own ties with Section 31 and then joined the crew as an unofficial Intelligence officer. ("Forbidden Crossing, Part II")

In 2386, Mayhem assited in researching the history of tribbles when the Phoenix-X came under attack by genetically modified killer-tribbles. ("Triple Tribble Sunday")

In 2390, after the crew of the Phoenix-X completed an undercover mission at Sigma Iota II, Mayhem and Wallace obtained Section 31 orders to stay behind and partake in the local Fizzbin Championships. ("Into Harshness")

In 2410, a radical Klingon named Treth tricked Lieutenant commander Melyot, of the USS Jenova, into acquiring and reprogramming Mayhem into supporting her cause. Through his modifications Mayhem was given false orders to join the crew of the USS Hijinx on an intel-collecting mission to the Delta Quadrant, when his real goal was to reprogram the Hijinx's deflector dish into opening a spatial interphasic flexure into what ended up being a Zalconian non-corporeal plane of existence. When Treth revealed her interference, Mayhem attempted to stop the deflector dish from being reconfigured but only succeeded in assisting it. ("Someone to Remember Them By")
